Visit our office to pick up a paper application. We are located at 501 Hart Street, Vincennes, IN 47591.
STEP 2
Fill out application form completely. Forms will not be accepted if answers are left blank. Background checks are required.
STEP 3
When your application is complete, call us at 812-882-5494 to schedule an application appointment. We do not accept applications via mail.
STEP 4
Return to our office on your appointment date to submit your application in person. We are located at 501 Hart Street, Vincennes, IN 47591. At your appointment, we’ll discuss your application and let you know of anything else required for eligibility. Placement can take between 6 months to a year.
